Dear Mr. Sam,

I would like to formally tell you at this moment that my daughter is not well, and I would like to take her to the hospital for a comprehensive medical checkup. I need to take a day's leave from work for this. She suffers from stomach pain and also has a fever. We are concerned about her. She hasn't gone to her school in the past week either.

I've spoken to Bob, and in my absence, he will meet with my clients and update you on the same status. After checking for your reference I will submit all of my daughter's medical reports.
